To speed your first visit, please download and print the New Patient Forms. Complete the forms and bring them with you to your initial visit along with your insurance card(s), if you have health insurance which may provide chiropractic benefits. For those with health insurance, an insurance release form is provided for general health insurance carriers with a separate form to be completed by Medicare recipients. Please complete and sign the insurance release form which applies to you so that our office may file your claims with your insurance carrier. Dr. Prestwich is a Preferred Provider with BlueCross/BlueShield of TN, CIGNA, PCHS Multi-Plan, and Medicare. For payment of services, Dr. Prestwich accepts Cash, Personal Checks and Visa, M/C, AmEx and Discover Credit Cards. |
